Your First Visit

Everything you need to know to prepare for your first appointment at Butura Oral Surgery

Your initial appointment will include a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed on the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to be scheduled.

What to Bring to Your First Visit

Referral Slip & X-Rays

Bring your surgical referral slip and any X-rays from your dentist or physician.

Insurance Information

If you have medical and/or dental insurance, bring your insurance card and any completed forms.

Medication List

A list of medications you take regularly, including any over-the-counter medications and supplements.

Medical History

A list of any medical conditions you have, including any allergies to medications.

Important Information

Minors

All patients under 18 years of age must be accompanied by a parent or legal guardian at the consultation visit.

Anesthesia

If you are to be sedated, it is required that you have a responsible adult drive you home after surgery and stay with you for the rest of the day.

Medical Conditions

Please alert our office to any medical conditions of concern, such as diabetes, high blood pressure, heart conditions, artificial joints, or if you are taking blood thinners.

X-Rays

If your dentist has taken X-rays, please have them forwarded to our office. If additional X-rays are needed, they can be taken at our facility.
